REGISTER
Each Delegate must submit a fully completed Registration Form and include the name(s) of any Guest(s) (adult or child) accompanying a Delegate.
Registration is carried out on a first-come, first-serve basis and will be closed as soon as capacity is reached, but in any event by 10 June 2009 (Forum's registration cut-off date).

In limited circumstances we will accept Registration Form in Word format

PAYMENT OF REGISTRATION FEE
The Registration Fee for a Delegate is $1,100.00. If a Delegate would like to bring a Guest, the Registration Fee for a Guest is $550.00.
The Delegate's Registration Fee includes transfer from the airport on arrival, all meeting facilities, simultaneous translation, catering during the meetings, Forum materials, all social events, and transportation to and from social events.
The Guest's Registration Fee includes transfer from the airport on arrival, all social events and transportation to and from social events, as well as the special Guest Program with city and cultural tours and lunches on 25 June and 26 June 2009.
The Kiev City Tour on 27 June 2009 (Saturday) is optional for Delegates and Guests at an extra charge (the registration for the optional tour will be carried out on-site during the first and second days of the Forum).
All payments must be made only in U.S. dollars by check or bank transfer within 7 (seven) days after submitting the Registration Form.

Please note that we shall communicate with the Delegates and Guests exclusively via e-mail. Therefore, in order to receive important logistical updates, please provide your accurate e-mail address as well as an alternate e-mail address in your Registration Form.
HOTEL BOOKING
HOTEL, TRAVEL COSTS, AND BOOKING ARE THE RESPONSIBILITIES OF THE DELEGATES.
For your convenience, we have reserved at special, considerably discounted Forum rates, a block of rooms at
Opera Hotel, which is the Forum venue.
These blocks will be released by Opera Hotel (i.e. no longer reserved for Forum participants) on 10 June 2009.
Please book your hotel accommodations as soon as possible. The number of rooms available at the Opera Hotel at special Forum rates is limited and these rooms will be available on a first-come, first-serve basis
We can also obtain preferential booking with a number of small, cost-efficient hotels, which are within 15-20 minutes'
walking distance from the Forum venue, Opera Hotel.

and we will put you in touch with the hotel.
BOOKING AND PAYMENT FOR HOTELS IS THE RESPONSIBILITY OF THE DELEGATES. The fact that you informed us of your intention to stay at a particular hotel does not constitute a booking.

10 June 2009 - to book accommodations with Opera Hotel (Venue of the Forum) at discounted rates subject to room availability.
10 June 2009 - to qualify for a partial refund of the Forum's Registration Fee (please see the Cancellation and Refund Policy below).

VISA REQUIREMENTS FOR UKRAINE
Citizens of CIS countries (except Turkmenistan) and a number of other countries, including member states of the European Union, Norway, Switzerland, Canada, the United States of America, and Japan do not require a visa to travel to Ukraine.
For all other countries we strongly recommend that you contact the Consular Section of the Embassy of Ukraine in your home country as soon as possible in order to check the visa requirements for citizens of your country.

CANCELLATION AND REFUND POLICY
All cancellations must be submitted in writing.
Cancellations must be received no later than 10 June 2009
to qualify for a partial refund, in which case the registration fee will be refunded minus an administrative fee of 10% ($110.00 for Delegate and $55.00 for Delegate's Guest) and any bank transfer charges (if applicable). If your cancellation notice is NOT received on or before
10 June 2009, we regret that we will be unable to refund the registration fee at all.
